Brought to you by Regional Medical Center

Every three years Regional Medical Center (RMC)–your local, non-profit, 501(c)(3) hospital–conducts a Community Health Needs Assessment for Delaware County.

Your opinions and health concerns in our community matter. As residents of Delaware County, RMC invites you to complete a brief, 8 question, community health needs assessment survey at regmedctr.org/2021CHNA. Hard copy surveys are also available at RMC’s Welcome Desk inside River Ridge Pavilion and at all Regional Family Health clinic locations.

“While this survey is a requirement for our healthcare facility, by doing this it provides us with very valuable information on what Delaware County residents feel are health concerns here locally,” shared Valerie Lindsay, Marketing and Fund Development Manager. “We really appreciate everyone’s participation and sharing this with others in the community.”

Survey results will be incorporated into hospital strategic planning and shared with other community resource organizations. For more information on the survey or to review previous Community Health Needs Assessment reporting, visit regmedctr.org/chna.

If you have any questions, please contact Valerie Lindsay at 563-927-7534 or vlindsay@regmedctr.org. Thank you in advance for your participation.
