Community Health Needs Assessment

Your health needs are our priority

The Patient Protection and Affordable Care Act requires 501(c)(3) hospital organizations to conduct a Community Health Needs Assessment (CHNA) at least once every three taxable years. We appreciate the time our community takes in helping us gather information on healthcare needs in Delaware County.

It’s time to start our process for the 2024-2025 Community Health Needs Assessment Survey. Your feedback will help identify health needs within Delaware County.

Once completed in the spring/summer of 2025, the 2024-2025 RMC and Delaware County Public Health CHNA Report and Implementation Plan will identify the results and strategies we will put in place to help improve the health and wellness of Delaware County. Together, we’ll make a difference!
