All referrals are evaluated based on the patient’s anticipated needs, the agency’s current caseload, staffing levels and if staff skills align with the patient’s needs. We do not currently provide daily visits, hourly or 24 hour care, or care in homes where there may be safety concerns.

Services Available
Homemakers assist with light housekeeping, grocery shopping, errands, meal preparation and more.
Occupational therapists provide in-home evaluation and treatment to help patients develop and maintain the ability to perform essential activities of daily living (bathing, dressing, preparing meals and more).
Physical therapists evaluate and provide rehabilitation to home-bound patients following an illness, surgery or injury. Care includes exercise programs, posture balancing training, daily living assessments, recommendation for home safety modification and more.
Speech therapists provide in-home evaluation and treatment for speech and swallowing problems including assistance in producing speech sounds, language comprehension and expression, along with swallowing techniques and diet recommendations.
Respite care gives rest and short-term relief for family members who are providing direct care of their loved ones. Home care aides provide assistance so errands can be run, shopping can be done or much needed rest can take place. Respite provides a social opportunity for your loved one, bringing a variety of relationships into their life.

Medicare, Medicaid, private insurance, sliding fee scale, long-term care insurance and private pay may cover home care. Once insurance coverage is approved, the primary insurance carrier is billed directly. Case management nurses work directly with you to coordinate benefits and provide appropriate, cost-effective care to meet your needs.
